Request Transcript

Only former students should complete this form. An official transcript must be sent directly from Bellarmine to a third party (e.g. school/scholarship organization/employer). It can be either mailed or emailed. Some recipients will not accept an official transcript via email. Please check prior to requesting your transcript. 

PLEASE NOTE: Only former students should use this form to request a transcript. If you are a third party institution (high school, university, etc) requesting a transcript on behalf of a former student, please request via email and submit a records request or a signed release. If you are the parent of a former student who is still in high school, please send an email to Melinda Michaelson, Registrar.

Requests are usually processed within two business days, except during school holidays. You will be contacted if there is a question regarding your request.
